To save time and money, could you download and complete the Health Upraisal Questionnaire before your 1st consultation please?

1st consultation 1 hour

Establish chief complaint, current signs and symptoms, current lifestyle practices and how they influence the symptoms.

• The physical assesment includes:
  Current level of acidity and toxicity
  Identify out of balance areas of the bodyUrinalysis test – pH balance
  Blood glucose test
  Indicant test – gut toxicity
  Zinc test

  Blood pressure reading and any other testing required such as Candida, Coeliac etc. (special testing are at extra cost)

2nd consultation 30 minutes
This is the first follow-up appointment to discuss and implemente:

• Tests results
• Initial Treatment Plan
• Questions and answers

3rd consultation and subsequent follow up appointments
These visits are designed to:

• Review long term wellness plan
• Review progress
• Support to establish and maintain healthy lifestyle practices to meet your desired wellness goal
